windows 8.1 open/save as dialog box components missing.

Took me about a day to finally narrow down my issue and it seems to have been caused by unchecky. Here are a few screenshots to explain what I mean in the title:

Running windows 8.1 pro build 9600 with latest windows updates as of 2015-07-20

Let me know if you want more info. I don't know why Unchecky would cause this sort of issue.
